Why Fix It⁉️ Wrist fractures most commonly involve the distal radius (forearm bone). A ##brokenwristmay or may not involve the radiocarpal joint, distal radioulnar joint, and/or the distal ulna. Wrist injuries are commonly from high-energy mechanisms in younger patients and lower energy mechanisms (falls) in older patients. Wrist fracture cause immediate wrist pain/swelling, and (if left untreated) can result in significant problems after healing. . 🛠️ Do You Need Surgery Distal radius fractures may have either surgical or non-surgical treatment. Non-surgical treatment requires acceptable fracture displacement, angulation, and shortening. Cast/Splint may be best option for some patients with the following: - <5mm shortening of Radius - <5-degree Radius inclination change - Articular step off <2mm - Dorsal angulation <5 degrees or within 20 degrees of the contralateral (other side) distal radius . ⌚️Displaced fractures should undergo closed reduction in an attempt to achieve an anatomic/acceptable alignment. Following closed reduction, arm is immobilized in a long-arm/sugar-tong splint to prevent pronation, supination, and elbow flexion (eliminating brachioradialis deforming force). Splints allow for swelling vs. cast. If post-reduction xrays show acceptable bone alignment, you can remain in splint and follow up with an ##orthopedicsurgeonfor regular xrays. If the reduction is not maintained, surgical intervention should be recommended. . 🔨 For Wrist fractures that do not meet acceptable alignment, the goal of surgical treatment is to achieve acceptable alignment and stable fixation for early motion. There are various methods of fixation, including pins, external fixators, dorsal plates, and a volar plate. Percutaneous pinning is useful in maintaining sagittal length/alignment (extra-articular fractures with a stable volar cortex). External fixation is often used in conjunction with percutaneous pin or plate fixation as it does not reliably restore the volar tilt on its own. Open-reduction internal fixation with plating can offer support to the subchondral bone to help maintain fracture reduction during healing.
